...to the Y2V Condition Management Programme (CMP)

This website is designed to inform visitors about the Condition Management Programme; to share information relating to the Programme; to provide answers to your questions; and to offer a forum for discussion and debate.

CMP has been running across South Yorkshire for the past 5 years alongside Pathways To Work, but as the new Work Programme is being introduced across the country we are now working with Yes2Ventures, based in Sheffield and colleagues in NHS Wales to deliver the programme in different regions across the UK

    Before I did CMP I had not worked for more than three years and I felt really low. I kept trying for jobs and got no response. CMP helped me to feel more confident. I made new friends, we worked in teams which was a great idea to do.

    Before CMP I was recovering from three lots of surgery, chemotherapy, radiotherapy, various scans and tests. I was feeling very tired, unsure of the future and very out of sorts with all the medication and anaesthetic etc.
