The
Condition Management Programme is an innovative programme delivered by the NHS
across South Yorkshire helping support individuals on
Incapacity Benefit/ESA to better understand and manage their
health condition with a view to them returning to work at
some point in the future. The programme is part of the
Pathways to Work initiative delivered by Jobcentreplus and
is currently being rolled out across the whole of England,
Scotland & Wales.
The Programme Consists of a 7 week core programme with
additional supporting modules and is mainly run in small
groups,
in local venues, such as leisure centres,
resource centres and libraries. The course covers
topics
like: Anxiety Management, Mood Management, Relaxation,
Pacing, Goal Setting, Lifestyle and much more. We also
deliver Pain Management, Sleep Management which individuals can undertake once
they have completed the Core Programme. We also run a
Fatigue Programme as an alternative to the Core Programme
for those individuals who require specific support regarding
living with fatigue.
Participation is totally voluntary and will not affect your
benefits in any way, if you decide to join the programme you
will be seen by one of our NHS 'health practitioners' in the
Jobcentre to inform you about the programme in your area and
decide with you if the programme will meet your
expectations.
If you join the programme we will pay for all your travel
expenses however you need to travel to get to the programme
venue, we will pay for any childcare or carer despite costs
you may have whilst attending and we will give
you a 3 or 6 month free complementary leisure pass to use
the leisure facilities in your local area. Also if you do
not have the appropriate clothing/ footwear to start using
the leisure facilities we have funds to reimburse reasonable
costs you may incur.
